Please fill out the form to register as an organ and tissue donor. By registering as a donor you consent to donate your organs and tissues at the time of your death. Organs and tissues will be recovered for the purpose of transplantation, however, in the event a donated organ or tissue cannot be used for transplant, an effort will be made to use the donation for research.

April is National Donate Life Month. During this month we reflect on the hope and life made possible through organ, eye, and tissue donation. The theme for this year’s celebration is the butterfly. The metamorphosis of the caterpillar to the butterfly represents the cycle of life and the process of transformation, growth, and renewal. The new life of the butterfly is a beautiful symbol of the Gift of Life given through donation.
